Scores 14+ suggest the likely presence of ADHD

Recommended frequency: Every 8 weeks to 6 months
Age 16+
In general, scores of 2 and above indicate areas for further inquiry.
The exceptions are questions related to suicide ideation (q 11), psychosis (q 12 &13), and substance abuse (q 21, 22 & 23) which should be investigated if scores are 1 or above.
Recommended frequency: Every 2 weeks
Age 12+
When screening for anxiety disorders, a recommended cut point for further evaluation is a score of 10 or greater.
0-4: none/minimal
5-9: mild
10-14: moderate
15-21: severe
Recommended frequency: Every 1 to 4 weeks
Age 16+
When screening for PTSD, a PCL-5 cut-point of 33 appears to be a reasonable value to propose until further psychometric work is available.
Research suggests a 5-10 point change represents reliable change and a 10-20 point change represents clinically significant change.
Recommended frequency: Every 4 weeks
Age 13+
When assessing panic, a score of 9 and above suggest the need for a formal diagnostic assessment.
0–1: none/mild
2–5: borderline
6–9: minimal
10–13: moderate
14 and above: severe
The composite score indicates, on average, how the test taker responded to each question, with higher scores indicating a higher severity of panic disorder.
Recommended frequency: Every 2 weeks
Age 12+
When screening for a depressive disorder, the following is recommended:
0-4: minimal
*client may not need depression treatment
5-9: mild
10-14: moderate
*use clinical judgement based on duration of symptoms and functional impairments
15-19: moderately severe
20-27: severe
*warrants treatment for depression
Recommended frequency: as needed, generally when suicide ideation is first noted
Age 16+
Recommended cutoff score of 7 and above. Indicates increased risk. Safety plan should be implemented.
